O’Donnell has been open about using Mounjaro, a GLP-1 medication for Type 2 diabetes also used by celebrities like Amy Schumer and Vanessa Williams. She started taking the medication in late 2022 and has lost more than 50 pounds over the past three years. The weight loss led to extra skin around her face and two lines that made her look sad, she explained.
Since relocating to Howth, Ireland, in 2025, strangers frequently asked if she was upset. “In Ireland, people would say, ‘Are you upset, darling? What’s the matter, love?’ And I’m like, ‘That’s just my face. I am not upset; it’s just how I look,'” she shared.

The “A League of Their Own” star went public with the news in a Substack essay titled “Decisions,” published on May 25, 2026. She followed up with before-and-after photos posted to Instagram on May 27. Speaking with E! News at the Tonys, she explained her reasoning for sharing such a personal choice.
“Authenticity is the goal in these days and times,” she said. “And I think all that matters is truth and love, and so I wanted to be truthful and say all the complicated emotions I had about it.”
In her Substack essay, O’Donnell admitted her face had begun “melting” after the weight loss. She also acknowledged she had once been firmly against facelifts. “I used to feel very strongly about facelifts. Not casually — morally. I had assigned myself as head of all women who would never — ever,” she wrote. “I thought it was a betrayal. Of feminism. Of aging. Of our team of women worldwide. And then I lost 50 pounds…”

The procedure O’Donnell chose was a lower deep-plane facelift, which lifts tissue under the facial muscles to avoid the skin looking too tight. Rather than transform her entire face, O’Donnell focused only on rejuvenating her lower face and neck.
At the Tony Awards, the mother of five wore a sharp black, gold and grey geometrically patterned suit, pairing the blazer and matching pants with taupe accents, glossy patent black shoes and black nails. O’Donnell removed her glasses on the carpet to show off her newly revitalized complexion as she chatted with reporters, posed for photographers and later shared snapshots from the night with fans on Instagram.
O’Donnell said her goal was to model bodily autonomy for her child, explaining that she didn’t want her child to learn that her body belonged to any ideology—even a well‑intentioned one like feminism. She added that the procedure left her looking like a slightly more rested, emotionally steadier version of herself.

O’Donnell also opened up about the conversations she had with her 13-year-old child, Clay, before going through with the procedure. “My little autistic 13-year-old said to me, ‘If you do it, I would not be able to respect you ever again,'” she recalled. “And that put me off it for a few months, but then when I went and did it in January, I came home 10 days later, and they never noticed.”
In her essay reflecting on the decision, the comedian wrote, “I didn’t disappear, I didn’t become someone else — I just stopped arguing with the mirror. And maybe that’s enough.”
